HIPAA compliance is not a feature you add before launch. It is a set of decisions about where PHI lives, who can touch it and how you prove it, made in the architecture, the code and the cloud account. We make those decisions with you from the first sprint.

CHALLENGES TEAMS FACE
Teams rarely fail HIPAA because they skipped encryption. They get caught by the PHI in a log file, the analytics script on a logged-in page, the vendor without a BAA or the risk analysis nobody updated. Here is where we see healthcare software teams get stuck, and how we get them unstuck.
A documented control set mapped to the HIPAA Security Rule safeguards, with architecture diagrams, data-flow maps, encryption and access evidence your sales team can hand to a security reviewer.
A gap assessment of code, cloud and vendors, then a prioritized remediation backlog: encryption, MFA, logging, session handling and secrets management fixed in order of risk.
Data-flow mapping that finds PHI in logs, error trackers, analytics tools, support tickets and test databases, then redaction, synthetic test data and vendor review to close each path.
Infrastructure as code on AWS, Azure or Google Cloud using only BAA-covered services, with encryption keys, network segmentation, backups and audit trails configured and checked automatically.
LLM and machine learning features built on BAA-covered model services, with PHI minimization, prompt and output logging, access control and human review designed in.
We build to today's rule and to the controls HHS has proposed (MFA, encryption, asset inventory, scanning and tested recovery), so your product is ready if the update is finalized.
OUR SERVICES
New products, existing products and the cloud and AI layers underneath them. We engineer the technical safeguards and produce the evidence your compliance program needs.
HIPAA-compliant software development for new products
Web platforms, mobile apps, patient portals, telehealth, remote monitoring and SaaS products designed for PHI from the first sprint, so security is part of the architecture rather than a pre-launch scramble.
HIPAA gap assessment and remediation of existing software
For products already in use: we review the code, the cloud and the vendors, rank the gaps by risk and fix them, while your product stays live.
HIPAA-eligible cloud architecture and DevSecOps
Cloud environments on AWS, Microsoft Azure or Google Cloud built from BAA-covered services and defined in code, so every environment is configured the same way and every change is reviewed.
Secure healthcare integration and APIs
PHI is most exposed when it moves. We build the interfaces between your product and EHRs, labs, payers and partners with authentication, encryption and logging on every connection.
AI and LLM features that handle PHI
Clinical summaries, ambient documentation, chat assistants and predictive models built so PHI stays inside BAA-covered services, is minimized before it reaches a model and is logged when it does.
Security testing and ongoing compliance support
Compliance does not end at launch. We test, monitor and update the product as threats, vendors and regulations change, including the proposed HIPAA Security Rule update.
Most teams know the HIPAA vocabulary. The difficulty is turning "encryption", "audit controls" and "minimum necessary" into specific decisions in a specific codebase, cloud account and vendor list, then proving it. QSS has engineered those decisions for healthcare clients for 16+ years, under ISO 27001 and CMMI Level 3 processes.
100+ healthcare projects, 20+ EHR, EMR and HIS integrations, and our own healthcare products, including DICOM/PACS Viewer and Remote Patient Monitoring. We know where PHI hides in a clinical workflow.

HIPAA describes what must be protected; it rarely says exactly how. We translate the Security Rule safeguards, the proposed 2025 update and the rules that apply to your data into specific controls, and can show you each one, control by control. Your organization remains responsible for its compliance program; we engineer and document the technical safeguards that support it.
Asset inventory, data-flow maps, threat and vulnerability findings delivered for your organization's HIPAA risk analysis and updated as the product changes.
AES-256 at rest and TLS 1.2+ in transit for databases, files, backups, messages and APIs, with managed keys and rotation.
Unique user IDs, multi-factor authentication, role-based minimum-necessary access, automatic logoff and break-glass with mandatory justification.
Tamper-resistant logs of every view, change, export and disclosure of PHI, retained to your policy and exportable for review.
An inventory of every service that touches PHI, built only on services covered by a BAA, with tracking scripts and SDKs reviewed for disclosure.
HIPAA-eligible environments on AWS, Azure or Google Cloud; encrypted backups, tested restores and documented recovery targets.
De-identification (Safe Harbor or Expert Determination), consent and 42 CFR Part 2 restrictions, and state health-privacy rules applied by configuration.
Vulnerability scanning in the pipeline and independent penetration tests, with findings tracked to closure and evidence kept for audits.
Best for a defined goal. An assessment, a remediation backlog or an MVP, with fixed price, milestone-based delivery and a clear finish date.
Best for an ongoing product. Named healthcare engineers, DevSecOps and QA who work as an extension of your team and keep the safeguards current.
Best when you want to own the capability long term. We build and run the team and platform, then transfer both to you on an agreed timeline.
We choose the stack for your requirements and your BAAs, not the other way round. These are the platforms and tools our healthcare and security teams use most.
Every product handles PHI differently, but buyers deserve a straight answer. These are typical ranges based on our delivery history and current market rates.

HIPAA-compliant software development is building or changing software that creates, receives, stores or transmits protected health information (PHI) so that it supports the HIPAA Privacy, Security and Breach Notification Rules. In practice that means a documented understanding of where PHI flows, and technical safeguards such as encryption, unique user IDs and multi-factor authentication, role-based access, audit logging, automatic logoff, integrity controls, secure hosting under a BAA and tested backup and recovery. Software alone cannot make an organization compliant: HIPAA also requires policies, training, risk analysis and business associate agreements. We build the technical safeguards and the evidence your compliance program relies on.
It depends on what the software does with PHI, how many systems it connects to and whether you are building new or fixing an existing product. Typical ranges: a readiness assessment $4,500–$18,000 over 2–4 weeks; a compliant MVP or remediation of an existing app $28,000–$120,000 over 3–5 months; a full platform with EHR integration and AI features $140,000–$380,000+ over 6–12 months. Building compliance in from the start costs less than retrofitting it later. You leave the discovery call with a range specific to your project.
A readiness assessment takes 2–4 weeks. A HIPAA-compliant MVP usually takes 3–5 months, and a full platform with integrations 6–12 months. The timeline grows with the number of EHR and partner integrations, the AI features involved and how quickly your organization can complete its own policies, risk analysis and vendor agreements.
HIPAA-ready platforms (low-code tools, backend services and communication APIs that sign a BAA) are a good route for internal tools and early pilots with standard workflows. Custom development makes sense when the product is what you sell, when you need integrations or AI features a platform does not support, or when per-user pricing grows faster than your business. Either way, you still need your own access controls, logging and risk analysis. Many of our clients combine both: custom application code on top of BAA-covered cloud and communication services.
Yes. We build HL7 v2 and FHIR R4 interfaces, usually through Mirth Connect or Iguana, to Epic, Oracle Health (Cerner), Meditech, athenahealth, Allscripts (Veradigm) and others, with authentication, encryption and logging on every connection. For apps that run inside the EHR we use SMART on FHIR, so access follows the EHR's own login and scopes. QSS has delivered 20+ EHR, EMR and HIS integrations.
No. HHS does not certify software, vendors or organizations as HIPAA compliant, and a "HIPAA certified" label is a marketing claim, not a government credential. What exists is your obligation to implement the required safeguards, perform and document a risk analysis, and be able to show OCR how you meet each standard. Some organizations add third-party assessments, but these are not HIPAA certification. We document every control we build so you can show your work.
HIPAA applies to covered entities (health plans, clearinghouses and most healthcare providers) and to business associates that handle PHI on their behalf. A direct-to-consumer health app that is not working for a covered entity is often outside HIPAA, but it may fall under the FTC Health Breach Notification Rule and state health-privacy laws such as Washington's My Health My Data Act. The answer depends on your contracts and data flows, so confirm it with counsel; we design the product so it can meet whichever obligations apply.
HHS published a proposed update to the HIPAA Security Rule in January 2025. It would remove the distinction between "required" and "addressable" safeguards and require, among other things, encryption of ePHI at rest and in transit, multi-factor authentication, a technology asset inventory and network map, vulnerability scanning at least every six months, penetration testing at least every 12 months, restoration of critical systems within 72 hours and annual compliance audits. As of October 2026 it is still a proposal, not a final rule: HHS's regulatory agenda targets a final rule around July 2027, and the current Security Rule remains in effect. We build to today's rule and, where practical, to the proposed controls, so your product does not need rework if the update is finalized.
When our team will create, receive, maintain or transmit PHI on your behalf, for example in production support or data migration, we sign a BAA before that access begins. Where possible we design the work so engineers use synthetic or de-identified data and never need production PHI. Your cloud, communication and AI providers need their own BAAs with you.
No. All three offer a BAA and a list of HIPAA-eligible services, but the cloud provider is responsible only for the infrastructure it runs. You remain responsible for using only covered services, configuring encryption, access, logging, networking and backups correctly, and protecting your application. We build cloud environments in code from BAA-covered services and check the configuration automatically on every change.
HIPAA allows two methods. Safe Harbor removes 18 specified identifiers (names, most geographic and date detail, contact details, record numbers and others) with no actual knowledge that the rest could identify someone. Expert Determination uses a qualified expert to confirm the risk of re-identification is very small, which keeps more useful detail. We build de-identification and tokenization pipelines for both methods and keep the re-identification keys separate and restricted.
Only through a service covered by a BAA with your organization and configured appropriately, such as Azure OpenAI, Amazon Bedrock or Google Vertex AI, or an open-weight model hosted in your own HIPAA-eligible cloud. Consumer chatbot accounts should not receive PHI. Even with a BAA, you still need minimum-necessary context, redaction, prompt and output logging, access control and human review, plus testing for accuracy and bias. We design those safeguards into every AI feature that touches PHI.
The technical safeguards are access control (unique user IDs, emergency access, automatic logoff and encryption), audit controls, integrity controls, person or entity authentication and transmission security. Some are "addressable" today, which means you must implement them or document why an equivalent measure is reasonable; the proposed update would make most of them mandatory. The rule also requires administrative and physical safeguards, such as risk analysis, workforce training and facility controls, which sit with your organization.
Yes. We start with a technical gap assessment of the code, cloud configuration, vendors and data flows, then deliver a prioritized remediation backlog and fix it in sprints while the product stays live. Typical fixes include encryption gaps, missing MFA, PHI in logs and analytics tools, weak session handling, unmanaged secrets and services without a BAA.
Log who accessed, created, changed, exported or shared PHI, when, from where and through which feature, without writing the PHI itself into the log. Logs should be tamper-resistant, monitored for unusual patterns and kept as long as your policy requires; HIPAA requires related documentation to be retained for six years. We build logging that serves security investigations, patient requests and auditors from the same source.
Current HIPAA rules require regular evaluation without fixing a frequency, and most enterprise customers ask for an annual penetration test. The proposed Security Rule update would require vulnerability scanning at least every six months and penetration testing at least every 12 months. We run automated scanning in every build and coordinate independent penetration tests, with findings tracked to closure.
Under the Breach Notification Rule, affected individuals must be notified without unreasonable delay and no later than 60 days after discovery; breaches affecting 500 or more people must also be reported to HHS within that period and, where 500 or more residents of a state are affected, to the media. Smaller breaches are reported to HHS annually. We build the detection, logging and incident-response runbooks that let your team establish quickly what happened, whose data was involved and whether notification is required.
